Texas Provider Audit Affidavits are legal documents used to certify the accuracy of a provider's records and accounts with the Texas Health and Human Services Commission (HHSC). The affidavits are typically used in response to an HHSC audit or in connection with a provider's appeal of an audit finding. The affidavit must be signed by a qualified accountant and is used to certify that the provider's books, records, and accounts have been reviewed and verified, and that they accurately reflect the financial activities of the provider. There are two main types of Texas Provider Audit Affidavits: Form H-1101 and Form H-1101A. Form H-1101 is used by providers who are receiving reimbursement from Medicaid for services provided to Medicaid recipients. Form H-1101A is used by providers who are not receiving Medicaid reimbursement, but who are appealing audit findings by HHSC.
