The checklist includes items such as the HMO's Certificate of Authority, Insurance Holding Company System Registration, an Actuarial Certification, a Form 8 and Schedule A (Rate Filing), and various other forms and documents. There are two types of Texas Transmittal Checklist for HMO Rate and Form Filings: one for initial filing and one for renewal filing. The initial filing checklist includes additional documents such as the HMO's Financial Statement and Rate Filing Instructions, while the renewal filing checklist includes documents such as the HMO's Financial Statement and Rate Filing Instructions, as well as a list of changes to the HMO's previously filed rates and forms.
