Texas Other Previous Hospital Affiliations (TO PHA) refer to the requirement that physicians licensed in the state of Texas must provide the Texas Medical Board with information regarding any hospital or clinic affiliations they had within the last five years in any other state. This information must include any hospital or clinic at which the physician has held privileges, been employed, or had any other professional relationship. TO PHA includes any medical facility, regardless of size, which provides care to inpatients or outpatients. This includes hospitals, clinics, medical centers, long-term care facilities, ambulatory surgical centers, and dialysis centers. It also includes any health care facility operated by a government agency.
