The Texas Notice of Maximum Medical Improvement and Estimated Permanent Impairment (MMI/EPI) is a form used by the Texas Workers’ Compensation Commission to document an injured employee’s medical condition and the estimated degree of impairment. The form is used to determine the employee’s eligibility for certain compensation benefits, such as temporary income benefits and medical benefits. The Texas MMI/EPI is divided into two sections: the Maximum Medical Improvement (MMI) section and the Estimated Permanent Impairment (EPI) section. The MMI section is used to document the employee’s medical condition and the date on which the employee’s condition is expected to reach maximum medical improvement. The EPI section is used to document the employee’s estimated degree of permanent physical impairment. The Texas MMI/EPI is issued by a physician who has examined the employee and determined the employee’s medical condition and estimated degree of permanent impairment. The physician must provide a complete and detailed report, including the employee’s medical history, current medical condition, and the estimated degree of permanent impairment. There are two types of Texas Notice of Maximum Medical Improvement and Estimated Permanent Impairment: the MMI/EPI-1 and the MMI/EPI-2. The MMI/EPI-1 is used for injuries that occur on or after September 1, 2005, and the MMI/EPI-2 is used for injuries that occur before September 1, 2005.
