It must also include the name and contact information of the person responsible for the safety program, and the date the cover sheet was completed. There are three types of TAPE Cover Sheets: the Basic TAPE Cover Sheet, the Advanced TAPE Cover Sheet, and the Self-Certified TAPE Cover Sheet. The Basic TAPE Cover Sheet is used for companies that have a safety program but do not have a written accident prevention plan. The Advanced TAPE Cover Sheet is for companies that have a written accident prevention plan. The Self-Certified TAPE Cover Sheet is for companies that have an internal safety program that meets the requirements of the Texas Department of Insurance.
