An Affidavit of Mailing (also known as a Certificate of Service) is a statement of fact, signed by the sender, that documents the mailing of a particular item. It is used primarily to prove that a legal document was sent to a certain individual or organization. There are two types of Affidavit of Mailing: Regular and Certified. A Regular Affidavit of Mailing includes a certificate that the document was sent via regular mail (or other means) and includes the date, to whom it was sent, and the type of delivery used. A Certified Affidavit of Mailing includes a certificate that the document was sent via certified mail, with a signature required upon delivery. The Affidavit of Mailing should be signed by the sender and notarized, and should include the date of mailing, the name and address of the recipient, the type of delivery used, and the signature of the sender.
