An appeal of a reconsideration is used when the individual disagrees with the SSA's decision after they have requested a reconsideration of the initial determination. The Complaint for Review of Social Security Decision must include the individual's name, Social Security Number, date of birth, address, telephone number, and a detailed explanation of why they disagree with the SSA's decision. The individual must also provide any supporting evidence they have that supports their claim that the SSA's decision was incorrect or unfair.
