Affidavit of Birth by Mother without a Birth Certificate: This affidavit is used when a birth certificate is not available. It includes the mother's sworn statement of the child's date and place of birth, the parents' names, and any other relevant information. 2. Affidavit of Birth by Mother with a Birth Certificate: This affidavit is used when a birth certificate is available, but additional proof is needed. It includes the mother's sworn statement of the same information as the affidavit without a birth certificate, as well as a notarized copy of the birth certificate.
