The Utah Initial Financial Report (Chapter 11 Cover Sheet) is a document filed with the Utah Division of Corporations & Commercial Code in order to provide financial information to the court during a bankruptcy proceeding. This document is used to inform the court of the debtor's financial situation and to provide a basis for the court to determine the feasibility of the debtor's reorganization plan. The Utah Initial Financial Report (Chapter 11 Cover Sheet) must be completed and filed with the court before the debtor's Chapter 11 reorganization plan can be approved. The document contains information such as the debtor's total assets, total liabilities, total net worth, estimated value of the debtor's assets, a list of creditors, and a statement of the debtor's income and expenses. There are two types of Utah Initial Financial Report (Chapter 11 Cover Sheet): the Short Form and the Long Form. The Short Form version is used when the debtor's total assets and liabilities are less than or equal to $2,000,000. The Long Form version is used when the debtor's total assets and liabilities are greater than $2,000,000.
