Report of Foreign Adoptees
The Utah Report of Foreign Adopters (RFA) is a document issued by the Utah Department of Health that provides a comprehensive record of foreign adoptions registered in the state. It includes information about the adopted child, the child's birth parents, the adoptive parents, and the court proceedings associated with the adoption. The RFA is the official record of a foreign adoption and is required to be completed and filed with the Utah Office of Vital Records for all foreign adoptions that take place in Utah. There are three types of Utah Report of Foreign Adopters: Full Report of Foreign Adopted, which includes the full name of the child, date of birth, place of birth, gender, adoptive parents, birth parents, court proceedings, and other details; Limited Report of Foreign Adopted, which includes the full name of the child, date of birth, place of birth, gender, adoptive parents, and court proceedings; and Summary Report of Foreign Adopted, which includes the full name of the child, date of birth, place of birth, gender, and adoptive parents. All three types of RFA must be completed and filed with the Utah Office of Vital Records to be recognized as a foreign adoption.
