The Utah Request for Verification of Employment (ROE) is a form that employers in the state of Utah used to verify the employment information of current or former employees. This form is typically used for purposes such as loan applications, background checks, and reference requests. It is important to note that this form is not a substitute for a job application or a job offer letter. There are two types of Utah Request for Verification of Employment forms. The first is the Standard ROE, which provides basic information such as the employee's name, job title, start and end dates of employment, pay rate, and an authorization from the employee allowing the employer to provide employment information. The second type of form is the Comprehensive ROE, which is used when more detailed information is required, such as a job description, reason for leaving, or other information regarding the employee's performance or qualifications. The Utah Request for Verification of Employment forms must be completed in full and signed by the employer before being sent to the requesting agency. The forms must also be accompanied by a copy of the employee's ID, as well as any other documents that may be required by the requesting agency.
