Utah Nomination For Oneself of Guardian and/or Conservator allows a person to nominate a guardian and/or conservator for themselves in the event they become unable to make decisions due to incapacity. This nomination is a written document that can be filed with the court that states the person's preferences for who should be appointed as guardian and/or conservator. There are two types of Utah Nomination For Oneself of Guardian and/or Conservator: (1) a nomination of guardian only, and (2) a nomination of both guardian and conservator. In either nomination, the person can name the same person or different people for each role. The nomination document must include the name and address of the nominated guardian and/or conservator, instructions regarding the guardian and/or conservator’s duties, and any other information the person wishes to provide. This nomination can be revoked at any time.
