A Utah Witness Affidavit (Minor Conservatorship) is a legal document used in the state of Utah that is required to appoint a conservator for a minor child. The document is signed by two witnesses in the presence of a notary public. It must be completed and signed by the conservator, the minor’s parents (or guardians) and two witnesses. There are two types of Utah Witness Affidavit (Minor Conservatorship): 1) the Basic Conservatorship and 2) the Supplemental Conservatorship. The Basic Conservatorship is used when a conservator is appointed solely for the care and management of the minor’s property. The Supplemental Conservatorship is used when a conservator is appointed for both the care and management of the minor’s property and person. The Utah Witness Affidavit (Minor Conservatorship) must include the name of the conservator, the name of the minor, the minor’s date of birth, the name of the minor’s parents or guardians, and the signatures of the conservator, the minor’s parents or guardians, and two witnesses. It must also include the date of the signing, the name and contact information of the notary public, and the notary public’s seal.
