A Utah Notice of Appeal is a legal document that is filed with the court to officially inform the court that a party wishes to appeal a decision made in a case. The notice must be filed with the court within thirty days after the entry of the decision or judgment that is being appealed. There are two types of Utah Notice of Appeal: a civil appeal and a criminal appeal. In a civil appeal, the party filing the notice is appealing the judgment of the court, and in a criminal appeal, the party is appealing a conviction or sentence. The Notice of Appeal must include the name of the court from which the appeal is being taken, the case number, the name of the party filing the appeal, a statement of the issues being appealed, and a statement of the relief requested. The Notice must also be signed by the appellant or the appellant's attorney.
