The Virginia Mediator Profile Form is a document used by mediators in Virginia in order to provide information about their qualifications and experience. It is used by the Virginia Supreme Court's Office of the Executive Secretary, which is responsible for certifying and appointing mediators in the state. The form is divided into four sections: Personal Information, Legal Education, Professional Experience and References. The Personal Information section includes basic information about the mediator, such as name, address, contact information and relevant qualifications. The Legal Education section requires the mediator to provide information about their education, including any degree or certification related to mediation or dispute resolution. The Professional Experience section requires the mediator to provide a written description of their experience related to mediation and dispute resolution. The References section requires the mediator to provide three references with contact information who can vouch for their qualifications and experience. There are two types of Virginia Mediator Profile Form: one for attorneys and one for non-attorneys.
