The Virginia Name Change Petition & Order — pgs. 7 & 8 of the Name Change Brochure is a set of forms that allows individuals to legally change their name in the Commonwealth of Virginia. This includes the Petition for Change of Name (Form DC-630) and the Order for Change of Name (Form DC-631). The Petition for Change of Name is the first step in the process. This form includes the petitioner’s current name, the new name they would like to use, the court where the case will be filed, and the reasons for the name change. The petitioner must also sign the form and have it notarized. The Order for Change of Name is the second step in the process. The form includes a judge’s signature and orders the name change to take effect. This form is notarized and filed with the court. Both forms are required for a successful name change in the Commonwealth of Virginia.
