Virginia Name Change Petition & Order — pgs. 13 & 14 of the Name Change Brochure is the legal document submitted to the court in order to request a name change in the state of Virginia. It must be filled out with the petitioner's current name, new name, county of residence, and the date of filing. The petitioner must also state the reasons for the name change and provide proof of identity. The form must then be signed in the presence of a notary public. After filing, the court will review the petition and may issue an order granting the name change. There are two types of Virginia Name Change Petition & Order — pgs. 13 & 14 of the Name Change Brochure: 1. Petition for Name Change — This form is used to request a name change and requires the petitioner to provide information including the current name, the proposed name, the county of residence, and the reasons for the name change. 2. Name Change Order — This form is issued by the court in response to the Petition for Name Change and is used to legally document the name change. It must be signed by the judge and notarized.
