The Virginia Name Change Petition & Order — Pgs. 19 & 20 of the Name Change Brochure is a form used to officially change a person’s name in the state of Virginia. It is a two-page document that provides an individual with a legal means to have their name changed on all government documents. The petition is divided into three parts: Part 1, Part 2, and Part 3. Part 1 is for the petitioner to fill out their current name and the name they wish to change it to, as well as any other relevant information. Part 2 is for the petitioner to provide details of their past criminal history, if any. Part 3 is an affidavit where the petitioner must swear or affirm that all the information provided is true and correct. The Order is a single page document that, once signed by an authorized judge, officially changes the petitioner’s name in the state of Virginia. It includes the petitioner’s old name, new name, and the date of the name change. There are two types of Virginia Name Change Petition & Order — Pgs. 19 & 20 of the Name Change Brochure: one for adults and one for minors. The adult petition and order are for individuals 18 years of age or older, while the minor petition and order are for individuals under the age of 18.
