The Vermont Payment Advices Cover Sheet is a document used by employers in Vermont to detail payments made to employees. It includes information about the employee, such as name, address, and Social Security number, as well as the employer’s name, address, and tax ID number. It also includes the date of the payment, the amount paid, and the type of payment (e.g., salary, wages, bonus, etc.). The Vermont Payment Advices Cover Sheet may be used to provide evidence for tax filing purposes or as proof of payment for other purposes. There are two types of Vermont Payment Advices Cover Sheet: the Vermont State Payment Advices Cover Sheet and the Federal Payment Advices Cover Sheet. The Vermont State Payment Advices Cover Sheet includes information about the employee and the employer as well as the date and amount of the payment. The Federal Payment Advices Cover Sheet also includes the same information, but also includes information specific to the Federal requirement, such as the Federal Employer Identification Number (VEIN).
