The Vermont Report of Mediation is a form used by mediators to document the outcome of a mediation session in the state of Vermont. The form is divided into three parts: (1) Mediator's Report, (2) Parties' Report, and (3) Settlement Agreement. The Mediator's Report includes a summary of the dispute, a description of the mediation process, and a summary of the outcome. The Parties' Report provides a summary of the positions taken by each party in the dispute. The Settlement Agreement contains the terms and conditions agreed to by both parties. There are two types of Vermont Report of Mediation: a standard form and a confidential form. The standard form is available to the public, while the confidential form is intended for use by the parties and mediator only.
