The Vermont Statement of Change of Address and/or Telephone Number and/or Other Contact Information is a document used by Vermont residents to update their contact information with the state. This document lets the state know of changes such as a new address, a change in telephone number, or other contact information. When a Vermont resident moves or changes their contact information, they are required to file this statement within 30 days of the change. There are two types of Vermont Statement of Change of Address and/or Telephone Number and/or Other Contact Information: 1. Form DVS-120: This form is used to update a Vermont resident's address, telephone number, and other contact information. This form can be filled out online or printed and mailed to the Department of Motor Vehicles. 2. Form DVS-121: This form is used to update a Vermont resident's address, telephone number, and other contact information for the purposes of voter registration. This form can be filled out online or printed and mailed to the Secretary of State.
