The Vermont Adult Guardian Closing Report and Discharge Order is a document completed at the end of a guardianship or conservatorship case in the state of Vermont. This document is filed in the court where the guardianship case was opened and includes information such as the guardian’s name and contact information, the ward’s name and contact information, the date of the closing report, and the guardian’s signature. The Discharge Order is then signed by the court, officially ending the guardianship. There are two types of Vermont Adult Guardian Closing Report and Discharge Orders: (1) a “Closing Report Without Discharge Order”, and (2) a “Closing Report With Discharge Order”. The Closing Report Without Discharge Order is used when the guardianship is being terminated, but the ward is not being discharged from the guardianship. The Closing Report With Discharge Order is used when the ward is being discharged from the guardianship and the guardianship is being terminated.
