The Vermont Affidavit in Support of Petition for Corrected Birth Certificate is a legal document used to make changes to a person’s official birth record. This form is used to correct information on a birth certificate that is incorrect, such as a misspelled name, incorrect dates, or other errors. The affidavit must be completed and signed by two witnesses who can confirm the accuracy of the information on the birth certificate. There are two types of Vermont Affidavit in Support of Petition for Corrected Birth Certificate: the Affidavit of Parentage and the Affidavit of Non-Parentage. The Affidavit of Parentage is used when the parent or legal guardian of the person whose birth certificate is being corrected is available to sign the affidavit. The Affidavit of Non-Parentage is used when the parent or legal guardian is not available to sign the affidavit. Both forms must be notarized by a Notary Public or other authorized official.
