The Vermont Parentage Affidavit (for the mother) is a legal document used to establish a mother's parentage of a child in the state of Vermont. This document is also known as an Acknowledgement of Parentage or Affidavit of Parentage. The Vermont Parentage Affidavit is a sworn statement that is signed in the presence of a notary public by the mother and the child's father. The mother must provide her name, address, place of birth, and the child's name, date of birth, and place of birth. The mother and father must also sign a statement that they are the parents of the child and agree to be legally responsible for the child. There are two types of Vermont Parentage Affidavit for the mother: an Acknowledgement of Parentage form and an Affidavit of Parentage form. The Acknowledgement of Parentage form is used when the mother and father are in agreement about the parentage of the child. The Affidavit of Parentage form is used when the mother and father are not in agreement about the parentage of the child.
