Vermont Medical Authorization is the permission given by a patient to another person or organization to access their medical information. This permission must be given in writing and signed by the patient or their legal representative. Vermont Medical Authorization allows for the release of medical records, as well as other confidential health information, to a designated recipient. This information may include lab results, diagnosis, medical history, treatments, and other medical-related information. There are two types of Vermont Medical Authorization: Standard Authorization and Limited Authorization. Standard Authorization allows a designated recipient to access a patient’s entire medical history, while Limited Authorization is more restrictive and only allows access to specific medical information. Both types of authorization require patient consent and must be renewed every 12 months.
