The Vermont Vocational Rehabilitation Disclosure Statement is an agreement made between the Vermont Department of Disabilities, Aging, and Independent Living (DAILY) and a person with disabilities who are eligible for Vocational Rehabilitation services. The purpose of the Disclosure Statement is to ensure that the individual is aware of their rights and responsibilities regarding their participation in the program. There are two types of Vermont Vocational Rehabilitation Disclosure Statement: the “Adult Disclosure Statement” and the “Youth Disclosure Statement”. The Adult Disclosure Statement is for individuals aged 18 and older who are being considered for vocational rehabilitation services. It covers topics such as the rights of the individual to be informed of their rights and responsibilities, to choose their own provider, and to receive services in a timely manner. The Youth Disclosure Statement is for individuals under the age of 18 and provides information on the rights and responsibilities that are associated with the program, including the rights of the individual to receive services in a timely manner and to be informed of any changes in the program. Both Disclosure Statements must be signed by the individual and by their parent or guardian before they can begin receiving services.
