The Washington Request For Reasonable Accommodation (Packet) is a form that enables individuals with disabilities to request accommodations in the workplace in Washington State. The packet is composed of two documents, the Request for Reasonable Accommodation (RFA) and the Interactive Process Guidelines (IPG). The RFA is used to make a formal request for accommodations and the IPG outlines the employer and employee responsibilities during the interactive process. There are two types of Washington Request for Reasonable Accommodation Packets: the General Packet and the Employee Packet. The General Packet is intended to be used by employers to request information about reasonable accommodations from a job applicant or employee. The Employee Packet is intended to be used by employees to request reasonable accommodations from their employer. Both packets provide guidance and information about the reasonable accommodation process.
