Washington Domestic Relations Information Form is a set of forms used to document and evaluate financial information in divorce, separation, or child support cases in the state of Washington. These forms are used to verify income and assets of both spouses, calculate child support, and divide assets in the event of a divorce. The forms include Declaration of Financial Assets and Liabilities, Income and Expense Declaration, Declarations of Real and Personal Property, Health Insurance and Child Care Costs Declaration, and Child Support Worksheet. There are also additional forms for special circumstances such as a Parenting Plan, Qualified Domestic Relations Order, and Qualified Medical Child Support Order.
