Washington Appendix 1 OF Dissolution is a form of dissolution of a domestic limited liability company (LLC) in the state of Washington. It is a legal document that must be filed with the Washington Secretary of State in order to dissolve a domestic LLC. The form is used to notify the state that the LLC is no longer active and will no longer conduct business. It must be signed by all members or managers of the LLC and must include a statement of dissolution, the name of the LLC, the address of the LLC, and the date of dissolution. There are two types of Washington Appendix 1 OF Dissolution: Voluntary Dissolution and Involuntary Dissolution. Voluntary Dissolution is when the LLC is voluntarily dissolved by the members or managers of the LLC. Involuntary Dissolution is when the LLC is involuntarily dissolved by court order.
