Washington Petition for Name Change (Individual) is a legal document used in the state of Washington to request a change of an individual's name. This document allows an adult or emancipated minor to change their name for a variety of reasons, such as marriage, divorce, adoption, or to restore a former name. The petition must be completed and signed in the presence of a notary public, and then filed with the Superior Court in the county of residency. There are two types of Washington Petition for Name Change (Individual): an Adult Name Change Petition and a Minor Name Change Petition. The Adult Name Change Petition is used by individuals 18 years of age or older, while the Minor Name Change Petition is used by minors who are either 16 or 17 years of age or emancipated minors. The petitioner must provide their current full name, birthdate, and residential address, as well as the proposed new name and the reason for the change. Supporting documentation may be required depending on the circumstances. Once the petition is filed, a judge will review the request and either grant or deny the name change.
