Washington Confidential Information Form (WAIF) is a type of disclosure form used to protect sensitive information in the state of Washington. It is also known as a "Certificate of Confidentiality Form". The WAIF is used to protect confidential information from public disclosure, intentional or unintentional use, or other unauthorized access. The form is issued by the Washington State Attorney General's Office and must be signed by the owner of the confidential information. There are two types of Washington Confidential Information Forms: the General Form and the Limited Form. The General Form is used to protect confidential information from all public disclosure, including disclosure to state and federal agencies. The Limited Form is used to protect the confidential information from public disclosure but allows for disclosure to a specific state agency or other limited source. Both forms must be signed by the owner of the confidential information and must include contact information, a description of the confidential information, and the name and title of the party receiving the form. The form must also be returned to the Washington State Attorney General's Office within 30 days of signing.
