Washington DOL Reporting Data Sheet is a form used by the Washington Department of Labor and Industries (DOL) to track and report statistical data related to workplace safety and health. This form is used to document the number of hours worked, the number of injuries and illnesses, and the costs associated with the workplace safety program. There are two main types of Washington DOL Reporting Data Sheet: the Employer's Accident and Illness Summary and the Employer's Annual Report. The Employer's Accident and Illness Summary is used to report information about workplace injuries, illnesses, and fatalities, as well as workers' compensation claims and costs associated with the workplace safety program. The Employer's Annual Report is used to report on the safety and health program for the year, including the number of workplace inspections, training and education, and other measures taken to promote workplace safety.
