Washington Notice of Change of Name or Address is a document used by Washington residents to notify the State of Washington of any changes to their legal name or address. This document is filed with the Washington Secretary of State’s office and is required for all individuals and businesses registered with the Washington State Business Licensing Service. There are two types of Washington Notice of Change of Name or Address: Change of Name and Change of Address. Change of Name forms are used to update the name of a registered individual or business with the Washington State Business Licensing Service. The form must be completed with the new name and signed by the individual or business representative. Additionally, a copy of the legal document(s) supporting the name change must be attached to the form. Change of Address forms are used to update the address of a registered individual or business with the Washington State Business Licensing Service. The form must be completed with the new address and signed by the individual or business representative. If the mailing address is different from the physical address, both must be provided.
