Washington Personal Care Plan — Report On Status of Person is a detailed report that outlines the needs and services required for an individual to remain safe and independent in their own home. It is developed by a qualified professional such as a social worker, nurse, or occupational therapist and is typically used to support people with disabilities and/or chronic illnesses who require assistance in their daily activities. The report is designed to provide a comprehensive overview of the individual’s current health and care needs, as well as their future care goals. The report is also used to evaluate the services and supports available to the individual and determine what services are necessary to maintain their current level of health and independence. There are three types of Washington Personal Care Plan — Report On Status of Person: Initial Plan, Reassessment Plan, and Transition Plan. The Initial Plan is used to create a baseline of the individual’s needs, assess their current level of health and functioning, and identify any potential service or support needs. The Reassessment Plan is used to review the individual’s current care plan and make any necessary changes. The Transition Plan is used to document the individual’s progress in transitioning from one level of care to another.
