The Washington Declaration of Proposed Guardian (GR-11) is a legal document used by the State of Washington to appoint a guardian to take care of the affairs of an individual who is unable to do so due to physical or mental incapacity. The document is used to name a person or entity who will be responsible for making decisions regarding the individual’s medical care, property, and other matters. The GR-11 document is primarily used for incapacitated adults, but can also be used for minors in certain circumstances. It provides the guardian with the authority to make decisions in the best interest of the individual, while also protecting their rights and interests. There are two types of Washington Declaration of Proposed Guardian (GR-11): the GR-11A and the GR-11B. The GR-11A is used to appoint a guardian for an adult who is not able to make decisions for themselves and who has not previously named a guardian. The GR-11B is used to appoint a guardian for an adult who has previously named a guardian, but the guardian is unable or unwilling to act.
