Washington Scheduling Questionnaire (WSU) is a questionnaire developed to assess the severity of workers' compensation claims in Washington State. It is used to determine an injured worker’s permanent partial disability, which is used to calculate the amount of benefits received from the State’s Department of Labor & Industries. The WSU consists of three sections: General Information, Impairment Rating, and Scheduling. The General Information section includes questions regarding the claimant’s job title, history of previous back injuries, and other personal information. The Impairment Rating section assesses the claimant’s physical impairment resulting from the work-related injury. The Scheduling section evaluates the claimant’s ability to perform certain occupational tasks and assigns a disability rating based on the evaluation. There are two types of Washington Scheduling Questionnaires: the WSQ-1 (for back injuries) and the WSQ-2 (for all other injuries).
