Washington Form 15. Statement of Arrangements is a form, created by the Department of Social and Health Services (DSS) in the state of Washington, that is used to establish legal guardianship or conservatorship. It is typically used when a person, due to age or disability, is unable to make decisions for themselves. There are two types of Washington Form 15. Statement of Arrangements: Guardianship and Conservatorship. Guardianship is the legal relationship between a guardian and the person they are appointed to care for, known as a ward. The guardian is responsible for making decisions for the ward, such as medical or financial decisions, and for protecting their welfare. Conservatorship is the legal relationship between a conservator and a person who is unable to manage their own affairs, known as a conservative. The conservator is responsible for managing the conservative's finances and assets, and for making sure their basic needs are met.
