Washington Criminal History Record (Cover Sheet) (Non-Parent Custody) is a document that provides a summary of an individual’s criminal history in the state of Washington. It is used to determine an individual’s eligibility for non-parent custody or adoption of a child and to assess the individual’s suitability for certain positions. The cover sheet contains information such as the individual’s name, date of birth, and a list of all Washington convictions and any convictions in other states. It also includes an overview of the individual’s Washington criminal history and a summary of all Washington arrests that did not result in a conviction. There are two types of Washington Criminal History Record (Cover Sheet) (Non-Parent Custody): Basic and Advanced. The Basic cover sheet provides basic information about the individual’s criminal history, while the Advanced cover sheet provides more detailed information about the individual’s criminal history.
