Washington Sealed CPS Information (Cover Sheet) is a document used by the Washington Department of Children, Youth, and Families (DCF) to protect the identities of children and families involved in child protective services (CPS) cases. This document is a record of the official information contained in the CPS file and is used to keep sensitive information from being disclosed to the public. The document is typically sealed with a court order and includes information such as the names of the involved parties, the address of the home, the name of the agency, the date of the investigation, and the outcome of the investigation. There are two types of Washington Sealed CPS Information (Cover Sheet): the Initial CPS Information (Cover Sheet) and the Final CPS Information (Cover Sheet). The Initial CPS Information (Cover Sheet) is used for the initial investigation of a CPS case and includes all the basic information needed to initiate the investigation. The Final CPS Information (Cover Sheet) is used to document the final outcome of the investigation and includes the findings, the recommended services, and the status of the case.
