Washington Sealed Financial Source Documents (Cover Sheet) is a form used to document the source of funds used in a financial transaction. This form is created and signed by the Washington State Department of Financial Institutions and is a requirement for any financial transaction involving the movement of funds within the state of Washington. The form is used to ensure that the funds are sourced from a legitimate source and are not being used for any illegal activities. There are two types of Washington Sealed Financial Source Documents (Cover Sheet): the Cash Transaction Cover Sheet (CCS) and the Cash and Non-Cash Transaction Cover Sheet (CNCT CS). The CCS is used to document transactions involving only cash, while the CNCT CS is used to document transactions involving both cash and non-cash sources of funds. Both forms require signatories from both parties involved in the transaction, along with a notarized signature from the Department of Financial Institutions.
