Washington Financial Statement — Harassment (Attachment) is a document used to record any financial statements related to instances of Harassment. This document is typically used by employers in Washington state as a means to document income, expenses, any claims, and any other related financial information related to Harassment incidents. This attachment is typically included with the Washington Financial Statement when filing a complaint of Harassment. There are two types of Washington Financial Statement — Harassment (Attachment): the Complainant Financial Statement and the Respondent Financial Statement. The Complainant Financial Statement is used to record any financial costs incurred by the Complainant as a direct result of the Harassment incident, while the Respondent Financial Statement is used to record any financial losses incurred by the Respondent due to the Harassment incident.
