Washington Proof of Mailing (Indian Child Welfare Act Notice) is a form used to document that a notice has been sent by mail to a person or agency in accordance with the requirements of the Indian Child Welfare Act (IOWA). This form can be used to document that a notice of removal from tribal jurisdiction, notice of hearing, notice of transfer of jurisdiction, notice of termination of parental rights, or other notice required by IOWA has been sent to the appropriate parties. There are two types of Washington Proof of Mailing (Indian Child Welfare Act Notice): 1) Service by Mail, which is used when the notice is sent by regular mail, and 2) Certified Mail, which is used when the notice is sent by certified mail with return receipt requested. The form includes fields to document the date the notice was sent, the name and address of the person or agency to whom it was sent, the type of notice sent, and other pertinent information.
