The Washington Petition for Approval of Budget, Disbursements and Initial Personal Care Plan (PT APR) is a document used by individuals in Washington State to request approval for the use of public funds to pay for long-term care services. The petition is submitted to the Washington State Medicaid Agency (DSS) and must be approved before any funds can be disbursed. There are three different types of Washington Petition for Approval of Budget, Disbursements and Initial Personal Care Plan (PT APR): 1. Initial Petition — This is the first petition individuals submit to the DSS, which includes the proposed budget and initial Personal Care Plan. 2. Recurring Petition — This petition is submitted on a recurring basis to the DSS and includes updates to the budget and Personal Care Plan. 3. Termination Petition — This petition is submitted when the individual no longer wishes to receive funds from the DSS, and includes the budget and Personal Care Plan for the period of time the petition has been in effect.
