The Washington Caregiver Report Cover Sheet is a form required by the Department of Social and Health Services (DSS) in order to process and track background checks for caregivers in Washington State. This form is used to indicate who is responsible for the background check, the date of the background check, and the type of background check being conducted. There are two types of Washington Caregiver Report Cover Sheet: 1. The Washington Caregiver Report Cover Sheet for Adults: This form is used to request a background check for an adult caregiver. It must be completed for any adult caregiver who will be providing services in the home of a vulnerable adult. 2. The Washington Caregiver Report Cover Sheet for Minors: This form is used to request a background check for a minor caregiver. It must be completed for any minor caregiver who will be providing services in the home of a vulnerable adult or child. Both forms must be completed and submitted to DSS in order to initiate the background check process. The forms are available online and must be signed by the requesting agency.
