Washington Community Truancy Board Agreement and Document Cover Sheet is a document used for truancy-related matters in Washington State. This document is used by the Washington Community Truancy Board to determine if a student is considered truant and to assess the best course of action for the student. It includes a description of the student’s behavior, the student’s current academic performance, any mitigating factors that may be relevant, and the student’s response to school-based interventions. It also includes a plan of action to address the student’s truancy and a signature page for the student and parents/guardians. There are two types of Washington Community Truancy Board Agreement and Document Cover Sheet: the Initial Agreement and the Follow-up Agreement. The Initial Agreement is used for the initial meeting with the student and their family to discuss the truancy issues. The Follow-up Agreement is used for subsequent meetings to review the progress of the student and the implementation of the plan of action.
