Extended Declaration of Entitlement: This Declaration provides additional benefits and protections beyond the Basic Declaration, including the right to receive benefits from the Department of Veterans Affairs, the right to have their Social Security Number kept confidential, and the right to receive services from the Department of Vocational Rehabilitation. 3. Comprehensive Declaration of Entitlement: This Declaration provides comprehensive benefits and protections including the right to receive benefits from the State's Employment Security Department, the right to receive services from the Department of Health, and the right to have their medical records kept confidential. These Declarations are issued by the Department of Social and Health Services and the State Department of Labor and Industries.
