The Washington Occupational Hearing Loss Questionnaire (WOULD) is a self-report questionnaire used to assess hearing loss in workers. It consists of four main sections: demographics, occupational history, hearing loss symptoms, and hearing problems. The questionnaire is designed to identify and quantify noise-induced hearing loss in workers, as well as to assess the impact of hearing loss on work performance and quality of life. There are two versions of the WOULD: the Standard WOULD and the Revised WOULD. The Standard version is a 15-item questionnaire that assesses the presence and severity of noise-induced hearing loss. The Revised version is a 24-item questionnaire that assesses the impact of hearing loss on work performance, communication, and quality of life. The WOULD is used as part of an occupational health evaluation to identify and quantify noise-induced hearing loss in workers. It is used to assess the severity of hearing loss, the impact of hearing loss on work performance, and the need for hearing protection and/or rehabilitation services.
