Washington Occupational Disease Work History is a record of employees’ exposure to workplace hazards and occupational diseases in the state of Washington. It is a mandatory requirement of the Washington Industrial Safety and Health Act (WISH) and applies to employers in all industries. The Work History includes information such as the employee’s name, job title, dates of exposure, and the hazardous substances or conditions that may have caused the occupational disease. The purpose of this record is to document potential exposures to occupational diseases and help employers identify and address potential safety hazards. There are two types of Washington Occupational Disease Work History: active and inactive. Active Work History includes records for current employees who are still working and have potential exposure to hazardous substances or conditions. Inactive Work History includes records for former employees who have been out of the workplace for at least one year and have a potential exposure to hazardous substances or conditions.
