The Washington CCP Treatment Report Form IV is a document used by medical professionals in the state of Washington to track the progress of patients receiving court-ordered treatment for substance abuse issues. It is used by certified counselors and/or treatment providers to report the results of treatment for court-involved individuals. The form is divided into five sections: Demographic Information, Treatment Plan, Client Progress, Treatment Completion, and Re-Evaluation. The Demographic Information section collects basic information about the client, such as name, date of birth, and address. The Treatment Plan section includes information about the treatment plan and goals that have been developed for the client. The Client Progress section captures information about the client’s progress in treatment, including attendance, participation, progress toward goals, and any recommended changes to the treatment plan. The Treatment Completion section summarizes the results of treatment and any recommendations for further treatment or follow-up. The Re-Evaluation section notes any future re-evaluations or assessments that are required. There are three different types of Washington CCP Treatment Report Form IVs, depending on the type of treatment being reported: Residential Treatment, Outpatient Treatment, and Evaluation/Assessment.
