The Washington Affidavit of Wages Paid Addendum B List of Next Tier Subcontractors — Public Works Contract is a state-mandated document used by prime contractors to identify the names and addresses of all subcontractors that have performed work under a public works contract. This document is signed by the prime contractor and serves as proof that all subcontractors on the project have received the prevailing wage as outlined in the applicable law. There are three types of Washington Affidavit of Wages Paid Addendum B List of Next Tier Subcontractors — Public Works Contract: 1) Standard Form, 2) Wage and Fringe Payment List, and 3) Minimal Wages Paid. The Standard Form includes the names and addresses of all subcontractors and their respective trade classifications that have performed work on the project. The Wage and Fringe Payment List includes the hours worked and the prevailing wages paid to each individual subcontractor. The Minimal Wages Paid document lists the minimum hourly rate of pay for each trade classification that was applicable at the time the contract was entered into.
